• Detailed Description of Project or Service 
• Detailed Project Budget or Projected Cost of Project or Service
• Pictures of existing structure the funds are intended to replace or repair, if applicable 
• Plans of replacement or repairs, if applicable 

 Incomplete forms will not be reviewed or considered. 

The Board of Directors meets once a month, we will contact you within 5 weeks of your request.
